Who this covers
This policy explains what information NextWave Wellness collects, how we use it, and the choices you have. It applies to this website, our online booking page, the appointment text messages we send, and the care we provide. In this policy, “we” and “us” mean NextWave Wellness.
Information we collect
- Contact details — your name, email address, and phone number, when you request an appointment or a membership, book online, or write to us.
- Appointment information — the service, date, and time you choose, any note you write when you book, and whether you have confirmed.
- Health information for your care — once you are a patient, the information we need to care for you: your history, exam findings, visit notes, test results, and treatment plans. If your provider asks to record part of a visit to help prepare your notes, we do so only with your permission.
- Membership and billing information — your membership tier, your Wellness Credit balance and how it is used, and what each visit cost and how it was paid. Our website and online booking page do not collect payment card numbers.
- Text-message information — your mobile number, whether and when you agreed to appointment texts, and any STOP or START replies. The texts we send contain only your first name and the day and time of your appointment.
- Website information — our contact form is delivered by a form service (Web3Forms), and our fonts are loaded from Adobe Fonts and Google Fonts, which can receive your IP address and browser details when a page loads.
How we use it
- Scheduling — to book, confirm, move, and cancel appointments and to reach you about them.
- Care — to document your visits, plan your treatment, and keep an accurate health record.
- Reminders — to send appointment confirmations and reminders by text, email, or phone.
- Billing and membership — to record what each visit costs and how it was paid, and to manage your credits and membership.
- Running the practice safely — to keep our systems secure, keep a record of who viewed or changed information, and meet our legal obligations.
Health information and HIPAA
As a healthcare provider, NextWave Wellness is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Health information we create or receive while caring for you is also governed by our Notice of Privacy Practices, which explains how that information may be used and disclosed and describes your rights over it. You can ask our front desk for a copy at any time. If this policy and the Notice differ about your health information, the Notice controls.
We do not sell your information
We do not sell your personal information or your health information, and we do not share it for advertising. We share it only with the people and companies that help us run the practice (for example, our scheduling and records systems and our text-message provider), who are bound to protect it; with others when you ask us to or give permission; and when the law allows or requires it, as described in our Notice of Privacy Practices.
Text messages
If you agree to appointment texts, we use your mobile number only to send appointment confirmations and reminders. We do not send marketing texts. We will not share your mobile number or your text-message consent with any third party for marketing or promotional purposes. You can stop the texts at any time by replying STOP. The full terms are in our SMS Terms.
How we protect it
Access to patient information is limited to the staff who need it for their role. Each person signs in individually, clinical staff and administrators use a second sign-in step, and we keep a record of who views or changes patient information. Information is encrypted while it travels over the internet. No system is perfectly secure, so we also ask you not to put details about your health in a text message or an email to us.
How long we keep it
We keep health records for as long as the law requires, and other information for as long as we need it for the purposes above.
Your choices: request, correct, or delete your information
You can ask us to give you a copy of your information, correct it, limit how we use it, or delete it. To make a request, email frontdesk@nextwave-wellness.com or call us at (308) 830-1951. We will confirm who you are before we release or change anything, and we will respond as quickly as we can and within the time the law allows.
Medical records are different: the law requires us to keep them for a minimum period, so we cannot delete a health record on request before that period ends. We can still delete other information we hold, such as an appointment request that never became a visit, and we will tell you what we can and cannot remove. You can turn appointment texts off at any time by replying STOP, or by asking the front desk.
Changes to this policy
If we change this policy, we will post the new version here and update the effective date above.
